Applicants shall completely fill out the Department of Public Safety's "Application for New/Renewal of a Firearms Identification Card or License to Carry Firearms." Please type or clearly print the responses and sign the application. An application is available at the Arlington Police Station or at any police department. (Click here for the application.)

Applicants must submit proof of citizenship. i.e., Birth or Naturalization Certificate.

Applicants must submit proof of residency.

Applicants for a New FID Card must submit verification that he/she has recently completed a firearms safety course approved by the Colonel of State Police, unless FID issued solely for Pepper Spray etc.
