Cemetery Report a headstone/marker in need of repair, landscaping issues, vandalism, or a general concern about cemetery property in Arlington.
Disease Report concern of public health issue (ex: West Nile Virus, Flu, Hepatitis)
Environmental Report an environmental concern such as asbestos, lead paint, dust or odor.
General Board of Selectmen Request Request information, ask questions or make a general comment to the Board of Selectmen
General Public Health Complaint Report of unsanitary conditions of a business or restaurant that may be violating health codes (trash, food, odor).
General Public Works Request information or make a general Public Works request.
Graffiti Report graffiti on public and/or private property.
Housing Inspection Request Report unhealthy living conditions or unsatisfactory housing facilities.
I Don't Know Place where residents can enter a request when they aren't sure who to send to.
Parks, Playgrounds, Bike Path Share a concern about a park, playground, field or bike path. Report vandalism.
Pests Report presence of animals, rodents, insects that may pose a health risk.
Recreation Facility Concern Share a concern about the Veterans' Memorial Skating Rink, Spray Pool, Reservoir Beach, or Gibbs Gym.
Recreation Other Request Make a general Recreation request or suggestion not described in other Recreation request forms.
Recreation-Tax Identification Number Request a Tax ID number for Recreation Programs attended.
Request for Public Health Information Request specific or general information about public health.
Request Recreation Information Request public information, report misinformation.
Reverse 911 Subscription Add and/or Remove your phone number(s) to Arlington's Reverse 911 Call Alert System.
Selectmen Agenda Item Request an item to be placed on the agenda for the next available Board of Selectmen meeting.
Snow & Ice Report missing snow plow service, icy conditions, snow plow damage or other snow and ice related issue.
Street & Road Maintenance Report potholes, uneven or damaged sidewalks, debris in street or other street and road maintenance issues.
Street Signs Report missing or damaged street and traffic signs or other sign related issue.
Trash & Recycling Report missed pick-ups, general trash or recycling concerns.
Trees Report fallen tree or limbs, damaged and diseased town trees, branches in wires or in need of trimming.
Update Town Meeting Member Contact A form for Town Meeting Members to update their contact information for the Clerk's Office and the online Town Meeting Member list.
Water & Sewer Report dirty water, street leaks, water damage, sewer back ups, odor, or other sewer and water issues.
Website Report broken links, missing files, or request content. Suggest a site enhancement or send general feedback on arlingtonma.gov or the Request/Answer Center.
Weights & Measures Report or inquire about food scales, taxi meters, gas pumps, heater meters orany consumer weights or measures.
